In a text box when you use embedded expression and if you accidently double click inside the text area (like if you were doing copy and paste) or highlighting text and double click it will cause the html markup to appear, sometimes lines overlapping. See thread below for example.
If this happens and you click on "Apply" it will remain there. So clicking on cancel is the only way to revert, which means losing any edits you were making.
Others confirm this does occur.
